Consent and Acknowledgment

By signing this form, I, the undersigned parent/guardian, grant permission for my child, ________________________, to participate in the Gaming Lock-In event hosted by The Tournament.

1. Medical Consent: In the event of an emergency, I authorize the event staff to administer basic first aid to my child and seek professional medical assistance if necessary. I understand that every reasonable effort will be made to contact me immediately in such cases.

2. Liability Release: I acknowledge and accept that participation in the lock-in event involves some inherent risks. I release The Tournament, its staff, and volunteers from any liability for injuries, damages, or loss sustained during the event, except in cases of gross negligence.

3. Behavior Agreement: I understand that my child must adhere to the event rules and follow instructions from staff and volunteers. I agree that inappropriate behavior may result in my child’s removal from the event, and I will be responsible for arranging immediate pickup if required.

4. Photography/Video Release: [Optional] I consent to the use of photographs or videos of my child taken during the event for promotional purposes on The Tournament's website, social media, or marketing materials.

5. Pickup Policy: I agree to pick up my child promptly at the event’s end time or designate the following individual(s) authorized to pick up my child:

NOTE: All authorized individuals must show a valid photo ID at the time of pick-up. For early pick up, parents MUST call from the parking lot.
